(1) My Indian passort name is different (e.g Lastname Firstname Kumar Middle).
(2) Birth Certificate (e.g Firstname LastName).
(3) Naturilization Certificate (e.g Firstname LastName). - They have followed Birth Certificate and not the passport. As well as nothing showing a stamp on the back of Naturalization Certificate indicating the name change.
How can I prove that all the names are same? As I have got query from Travisa. Please help.

First my Indian passport was created on May 28th 1987. Later wanted birth certificate for some other reason but when I was born had no name in the Birth Certificate, so my parents had to do affidavit for my name but it was not done according to my Indian passport name. During Naturalization they (US Naturalization) took the name from Birth Certificate and not according to the passport. As well as the middle name was dropped during the Naturalization. I had discussion with the officer during the interview but they said we will follow the birth certificate. So the name between the Indian passport and the naturalization does not match.
Because of the above reason I am submitting an affidavit saying that indeed all the name below are for the same person First Name Last Name. I have officially no name change and don't have any documents like court order, a stamp on the back of Naturalization certificate indicating the change, a copy of marriage certificate and a US and Indian newspaper advertisement of the name change. Please accept this affidavit.
